Job summary

The Department of Economic Security, Division of Developmental Disabilities, seeks a Case Manager 2 in Lakeside, Arizona to support individuals with developmental disabilities and their families. You will drive to member homes, schools and program sites to assess needs and develop Person-Centered Service Plans.

Responsibilities include coordinating services, writing progress notes, and collaborating with community agencies.

Qualifications

• Knowledge of methods, procedures, and techniques used in PCSP development.
• Familiarity with state/federal statutes relating to developmental disabilities.
• Principles and practices of case management.

Responsibilities

• Drive a vehicle to conduct onsite visits to members’ homes, schools or sites.
• Develop, review, monitor and evaluate PCSP and document progress.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families.
• Write progress notes and maintain case records; prepare reports.
