Job summary

Department of Economic Security (DES) seeks a Case Manager 2 - Targeted Unit in Prescott Valley, AZ. The role involves managing care plans, coordinating services, and supporting families of individuals with developmental disabilities.

The incumbent will travel for on-site visits and collaborate with partners to ensure timely, person-centered supports. Strong knowledge of HIPAA, case management principles, and PCSP development is essential.

Qualifications

• Must have HIPAA knowledge and familiarity with health and behavioral health terminology.
• Experience developing and reviewing Person-Centered Service Plans (PCSP).
• Knowledge of state and federal statutes related to developmental disabilities and care.
• Strong case management skills and ability to organize caseloads and maintain records.
• Ability to work independently and within diverse teams while maintaining confidentiality.

Responsibilities

• Drive to member homes, schools, or program sites for onsite visits and relationship building.
• Develop and review PCSP; document progress and referrals to services as needed.
• Facilitate meetings and visits to support member/family decisions and service referrals.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes, maintain case records, and prepare reports and special projects.
