Job summary

Arizona Department of Economic Security, Division of Developmental Disabilities (DDD), seeks a Case Manager 3 to support individuals with developmental disabilities in Chandler and Phoenix, delivering person-centered services and coordinating supports.

Daily duties include driving to visits, developing PCSPs, completing referrals, and collaborating with community partners. The role requires RN license or relevant degree with experience, and offers a comprehensive benefits package.

Qualifications

• HIPAA and patient privacy knowledge.
• Knowledge of PCSP development methods.
• Familiarity with federal and state statutes relating to developmental disabilities.
• Principles and practices of case management.
• Ability to work independently and in teams.

Responsibilities

• Drive to member homes/sites to assess service needs.
• Develop and monitor PCSP; refer to community services.
• Facilitate meetings; assist with member/family choices and referrals.
• Collaborate with agencies to support members and families in crisis.
• Maintain case records; prepare reports; attend meetings.
