Job summary

Department of Economic Security (DES) seeks a Case Manager 2 to join the Division of Developmental Disabilities in Tucson, AZ. You will support individuals with developmental disabilities through PCSPs, referrals, and coordination with community partners.

Responsibilities include driving to member sites, writing progress notes, and maintaining case records while ensuring timely service delivery. AZ residency and background checks are required; competitive benefits offered.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Special Education or Counseling with relevant experience.
• RN license in good standing OR Bachelor's in SW/related field with 1 year case management experience.
• Driver's license and background checks required.

Responsibilities

• Drive to members' homes, schools, or sites to assess service needs and supports.
• Develop, review, and monitor Person-Centered Service Plans (PCSP).
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families in crisis.
