Job summary

The Department of Economic Security (DES) in Arizona is seeking a Case Manager 2 for the Division of Developmental Disabilities in Tucson to support individuals and families toward a better quality of life.

You will conduct on-site visits, develop and monitor Person-Centered Service Plans, collaborate with community agencies, and document progress while ensuring timely referrals and reporting.

Qualifications

• Knowledge of Person-Centered Service Plans (PCSP).
• Understanding of federal/state statutes and agency policies for developmental disabilities.
• Experience applying case management principles.

Responsibilities

• Drive a vehicle to conduct onsite visits to members' homes, schools, and program sites.
• Develop, review, monitor and evaluate PCSP and document progress.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and maintain case records, and prepare reports.
