Job summary

Arizona Department of Economic Security (DES) seeks an experienced Case Manager 2 to support individuals with developmental disabilities in the Chandler area. The role involves visiting members, developing and evaluating PCSPs, and coordinating services with families and community agencies.

The candidate will document progress, prepare reports, and ensure compliance with timelines. A valid driver’s license and background checks are required, with bilingual Spanish/English a plus.

Qualifications

• Knowledge of HIPAA and health terminology.
• Knowledge of Person-Centered Service Plans (PCSP).
• Understanding federal and state policies for developmental disabilities.
• Principles of case management.

Responsibilities

• Drive to member homes, schools or sites to assess needs.
• Develop, review and monitor PCSP with timely documentation.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families.
• Document progress notes and prepare reports.
