Job summary

The Arizona Department of Economic Security, Division of Developmental Disabilities, is seeking a Case Manager 3 to join a team that supports individuals with developmental disabilities and their families. The role focuses on developing Person-Centered Service Plans, coordinating services, and engaging with community partners to promote independence and quality of life.

You will drive to member homes, maintain confidential records, and participate in referrals and team meetings while ensuring

Qualifications

• Arizona licensed RN in good standing or Bachelor in Social Work, Psychology, Special Education or Counseling with 1 year case management experience.
• Bachelor’s in Human Services or related field with 1 year case management experience or 3 years work with ID/DD.
• Valid driver’s license and pass background checks.

Responsibilities

• Drive to members’ homes, schools, or sites to build relationships and discuss needs.
• Develop, review, and monitor Person-Centered Service Plans (PCSP) and document progress.
• Facilitate meetings to support member/family choices and complete referrals.
• Collaborate with community agencies to assist members and families in crisis.
• Maintain case records and prepare various reports; attend staff meetings.
