Job summary

State of Arizona's Department of Economic Security - DES, Division of Developmental Disabilities (DDD), seeks a Case Manager 2 to support Arizonans with developmental disabilities by coordinating services, building plans, and ensuring timely advocacy and community engagement.

The role requires compliance with licensing and background checks, a valid driver’s license, and a bachelor’s degree or RN licensure with relevant experience. Work is performed in Lakeside, AZ with a strong team focus.

Qualifications

• Arizona licensed Registered Nurse OR Bachelor Degree in Social Work, Psychology, Special Education or Counseling with at least 1 year of case management experience.
• Bachelor’s Degree in Human Services or related field with at least 1 year of case management experience OR at least 3 years’ experience with individuals with ID/DD or SMI.
• Driver’s license and background checks required.

Responsibilities

• Drive to members' homes, schools, and program sites to build relationships and assess service needs.
• Develop, review, and monitor PCSP and refer services.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families in crisis.
• Document progress notes and reports.
