Job summary
The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking a Case Manager 2 in Lakeside, Arizona. You will help individuals with developmental disabilities and their families access flexible, high-quality services and supports in the community.
Key duties include driving to member homes and sites, developing PCSP, coordinating referrals, and collaborating with community agencies.
Qualifications
- Arizona licensed RN OR Bachelor’s degree in Social Work, Psychology, Special Education or Counseling with at least 1 year of case management experience.
- Alternatively, a Bachelor’s in Human Services or related field with 1+ year of case management experience for elderly or disabled individuals.
- Must pass background checks and have fingerprint clearance.
Responsibilities
- Drive a vehicle to member homes and program sites to assess service needs.
- Develop, review, and monitor PCSP with timely documentation.
- Facilitate meetings and referrals for services.
- Collaborate with community agencies to support members and families.
- Prepare reports and maintain case files with required timelines.
