Job summary

The Department of Economic Security (DES) is seeking a Case Manager 2 - Targeted Unit for the Specialty District serving Central and West Arizona. The role involves driving to members' homes, schools, and program sites to assess needs and discuss services, and to develop comprehensive plans with timely documentation.

Qualifications include a Bachelor's in SW/ Psychology or related field, or RN license with case management experience, plus a valid driver's license and background checks.

Qualifications

• Knowledge of HIPAA and health/behavioral health terminology.
• Experience developing Person-Centered Service Plans (PCSP).
• Familiarity with state/federal statutes related to developmental disabilities.

Responsibilities

• Drive to members' homes/schools/sites to assess needs and discuss services.
• Develop, review, and monitor PCSP with timely documentation.
• Collaborate with community agencies to support members and families in crisis.
