Job summary

The Department of Economic Security (DES) is seeking a Case Manager 2 in Sierra Vista, AZ to provide person-centered service planning and support for individuals with developmental disabilities and their families.

Essential duties include on-site visits, referrals, and collaboration with agencies to ensure timely, quality services. Applicants must meet licensure/education requirements and pass background checks.

Qualifications

• Bachelor’s degree requirements include SW/psychology/special education or counseling; experience in case management for elderly and/or disabled and/or SMI.
• RN license in good standing or Bachelor degree in Human Services/Behavioral Science with relevant experience required.
• Driver’s license and background checks required for state service employment.

Responsibilities

• Drive to members' homes, schools, or program sites to discuss service needs.
• Develop and monitor PCSP with timely documentation and referrals.
• Facilitate meetings and assist members and families with choices and services.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and prepare reports and special projects.
