Job summary

DEPARTMENT OF ECONOMIC SECURITY seeks a Case Manager 2 in Chandler/AZ. The role involves traveling to members’ homes and program sites to assess needs and support community-based services.

You will develop PCSPs, coordinate referrals, and collaborate with agencies to assist families. Required are a nursing license in AZ or a relevant degree with experience in case management for elderly or disabled individuals, plus background checks and driving.

Qualifications

• Knowledge of HIPAA regulations and health/privacy standards.
• Experience in case management for elderly or disabled individuals.
• Ability to interview, counsel, and coordinate services with families.
• Familiarity with PCSP processes and state/federal policies.

Responsibilities

• Drive to members' homes, schools, or program sites to assess needs.
• Develop, review, and monitor Person-Centered Service Plans (PCSP).
• Facilitate meetings and referrals for services and supports.
• Collaborate with community agencies to assist members and families in crisis.
• Document progress, maintain case records, and prepare reports.
