Job summary

DEPARTMENT OF ECONOMIC SECURITY seeks a Case Manager 2 - Targeted Unit for the Specialty District Central/West in Avondale, AZ. Responsibilities include site visits, PCSP development, and coordinating referrals with community partners.

Ideal candidates will have relevant degrees and experience in case management for individuals with developmental disabilities, SMI, or elderly populations. Atomic driving and RN/SW credentials not required but considered assets.

Qualifications

• Knowledge of HIPAA compliance and privacy rules.
• Understanding of medical and behavioral health terminology.
• Experience developing Person-Centered Service Plans (PCSP).
• Familiarity with federal/state statutes and agency policies for developmental disabilities.
• Knowledge of principles and practices of case management.

Responsibilities

• Drive to visit members at home/school/sites to discuss service needs.
• Develop, review, and monitor PCSPs and document progress.
• Facilitate meetings to assist member/family choices and referrals.
• Collaborate with community agencies to provide support during crises.
• Write progress notes, maintain case files, and prepare reports.
