Job summary

The Department of Economic Security (DES), Division of Developmental Disabilities (DDD) in Yuma, AZ, seeks a Case Manager 2 to deliver person-centered supports and ensure timely service planning. You will drive field visits, document progress, and collaborate with families and community partners to connect services.

Required qualifications include a related bachelor's degree or RN/licensed, relevant case management experience, and a valid driver's license; bilingual in Spanish is a plus.

Qualifications

• Bachelor's degree in Social Work, Psychology, Special Education, Counseling, or equivalent.
• Experience providing case management to elderly or disabled individuals or those with serious mental illness.
• Valid driver's license and background checks required.

Responsibilities

• Drive to members' homes and program sites to assess service needs and supports.
• Develop, review, monitor and evaluate PCSP and document progress.
• Facilitate meetings and referrals for assessed/covered services.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and maintain case records; prepare reports and special projects.
