Job summary

The Arizona Department of Economic Security, Division of Developmental Disabilities, is seeking a Case Manager 3 in Flagstaff to support individuals and families through person-centered planning and service coordination.

You will drive to member homes, develop PCSPs, collaborate with community partners, document progress, and participate in staff meetings. Requires degree (SW/psychology) or RN license and a valid driver’s license.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Special Education or Counseling, or RN license.
• 1–3 years experience in case management for elderly or disabled individuals.
• Valid driver’s license and clearance checks required.

Responsibilities

• Drive a vehicle to conduct onsite visits to members’ homes, schools and program sites.
• Develop, review, monitor and evaluate PCSP; complete referrals to services.
• Facilitate meetings and program visits to assist member/family choices.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and maintain case records; attend meetings and trainings.
