Job summary

Department of Economic Security is seeking a Case Manager 3 – Targeted Unit in the Division of Developmental Disabilities (DDD), serving the Specialty District East in Apache Junction, AZ. The role requires driving to member homes, developing PCSPs, and coordinating services with community partners.

Candidates must meet specific licensure or degree requirements and pass background checks. The position offers a salary of $53,000.00–$55,000.00, with a Grade 18, and a closing date of 2026-10-12.

Qualifications

• Arizona-licensed RN or BS in Social Work/related field with case management experience.
• Bachelor's in Human Services or related field with at least 1 year of case management experience, or 3 years in ID/DD.
• If driving is required, possess a valid driver’s license and auto insurance; complete driver checks.

Responsibilities

• Drive to members' homes, schools, and program sites to build relationships and assess needs.
• Develop, review, and monitor Person-Centered Service Plans (PCSP) and coordinate referrals.
• Facilitate meetings and visits to assist with member/family choices; complete referrals for services.
• Collaborate with community agencies to support members and families in crisis.
• Maintain case records and prepare reports; attend meetings and trainings; may act as supervisor in absence.
