• Understanding of community resources and health/behavioral health topics.
• Ability to maintain confidentiality and work with diverse teams.

Responsibilities

• Drive to members' homes for on-site visits and develop support referrals.
• Provide crisis support and attend meetings with families and providers.
• Assess eligibility and document progress in case records and reports.
• Lead or support teams in absence of supervisor.
