Job summary

The role requires an active WA RN or Social Worker license, a master’s in social work or BSN, and 3+ years clinical experience. Strong communication and independent work skills are essential.

Qualifications

• Master’s degree in social work or bachelor’s degree in nursing (preferred).
• Current, unrestricted license in WA as RN or Social Worker (required).
• Min 1 year case management, home health or discharge planning experience (required).
• Min 1 year behavioral health experience preferred.
• Min 3 years clinical experience in acute or outpatient setting (required).
• Proficiency with Microsoft Office products.

Responsibilities

• Performs telephonic case management for members with acute, chronic, and complex needs.
• Conducts comprehensive member assessments to identify needs and risks.
• Develops and updates individualized care plans with the care team.
• Coordinates care across providers and community resources to ensure access and continuity.
• Advocates for members and addresses barriers to care and access to services.
• Monitors progress and modifies interventions as needed; participates in care team activities.
• Documents all activities in the care management system per policy and regulatory standards.
• Implements care management programs and quality initiatives with stakeholders.
