Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for the Care at Home program in Washington. The role blends field-based in-home assessments with telephonic care management for Medicaid LTSS and DSNP members across WA and CO.

The RN will coordinate care with interdisciplinary teams, set care goals, document in EMR, and support transitions of care while addressing clinical and social needs in patients’ homes.

Qualifications

• Active unrestricted RN licensure in state of residence and/or ability to obtain licensure to support Care at Home markets.
• 2+ years of experience as a Registered Nurse.
• Experience with Medicaid Long Term Care, LTSS, Home-and Community-Based Services, or related Medicaid care management program.
• Proven ability to work with diverse care teams and in non-clinical settings (homes).
• Ability to travel up to 75% of the time; valid driver's license and reliable transportation.

Responsibilities

• Conduct in-home assessments, care coordination visits, and community-based member engagement (≈75% of time).
• Provide telephonic case management and care coordination for Washington and Colorado markets (≈25% of time).
• Assess health status within licensure scope and model-of-care cadence; establish care goals.
• Plan, implement, and evaluate responses to the plan of care with interdisciplinary teams.
• Assist with transitions between care settings in collaboration with PCPs and other providers.
• Document accurately in the electronic medical record and educate patients and families.
