Job summary

UnitedHealth Group's Optum Care at Home is hiring a Telephonic RN Case Manager to serve as primary contact for members with complex needs, conducting telephonic assessments, creating care plans, coordinating transitions, and addressing barriers to care in a high-volume, patient-centered environment.

You'll perform remote, telephone-based case management across Mountain and Pacific Time Zones, require RN licensure, and bring strong documentation in EHRs, with a preference for Washington or

Qualifications

• Active, unrestricted RN license with authority to practice in WA and CO by start date.
• 2 years of professional RN experience.
• Experience coordinating care for patients with complex/ high-risk needs.
• Experience with Medicaid LTSS or similar home-based care programs.
• Experience in case management or transitions of care.
• Proficiency documenting in electronic health records and care management platforms.
• Driver’s License and reliable transportation.

Responsibilities

• Conduct telephonic assessments of members’ medical, behavioral health, functional, psychosocial, and socioeconomic needs.
• Develop, implement, and reassess person-centered care plans reflecting goals and risks.
• Coordinate care across Medicare, Medicaid, primary/specialty care, and community programs.
• Identify barriers to treatment and connect members with resources.
• Facilitate safe transitions between care settings and ensure timely follow-up.
• Document assessments, plans, interventions, and outcomes accurately in systems.
• Maintain RN licensure and required credentials.
