Job summary

UnitedHealth Group seeks a Behavioral Health Case Manager to deliver person-centered care in Washington with in-home visits and telephonic support for WA and CO members. You will coordinate services across medical, behavioral health, and community settings while building care plans for high‑risk individuals.

Role requires a Master's in social work or related field, active WA licensure, and 2+ years in healthcare, plus 1+ year in clinical/case management.

Qualifications

• Master's degree in social work or related behavioral health field.
• Active Washington behavioral health licensure (LICSW/LCSW/LMSW or equivalent).
• 2+ years in a healthcare setting.
• 1+ years of clinical or case management experience.
• Experience with Medicaid LTSS or related programs.
• Ability to travel up to 75% within Washington state and hold a valid driver’s license.

Responsibilities

• Conduct in-home and telephonic behavioral health assessments.
• Develop and manage individualized care plans for high‑risk members.
• Coordinate with providers for treatment planning and care coordination.
• Review treatment requests to support utilization and medical necessity determinations.
• Educate members about Medicaid/Medicare benefits and access to services.
• Document assessments and interactions in electronic systems.
