Job summary

Community Health Plan of Washington is seeking a Case Manager I to oversee telephonic case management for CHPW members with diverse needs in a fully remote role that prioritizes Washington residents. Active WA RN or Social Work licensure is required.

The position emphasizes care coordination, development of individualized care plans, and collaboration with an interdisciplinary team to improve outcomes and control costs. Proficiency in MS Office and relevant experience are essential.

Qualifications

• Master’s degree in social work or Bachelor’s in nursing.
• Current, unrestricted license in WA as RN or Social Worker.
• Minimum 1 year case management/home health/discharge planning.
• 1 year behavioral health experience preferred.
• 3+ years clinical experience in acute care/outpatient.
• Proficient with Microsoft Office.
• Case Management Certification preferred.
• Bilingual abilities preferred.

Responsibilities

• Perform telephonic case management for members with acute, chronic, and complex needs.
• Conduct comprehensive assessments of clinical, psychosocial, and social determinants of health needs.
• Develop and monitor individualized care plans with the care team.
• Coordinate care across providers, community resources, and internal teams.
• Advocate for members and address barriers to care.
• Monitor progress and modify interventions as needed.
• Participate in interdisciplinary care team activities and meetings.
• Document assessments, care plans, and progress in the care mgmt system.
• Implement programs and quality initiatives with stakeholders.
• Serve as a clinical resource and promote integrated care coordination.
• Contribute to team training and other priorities as needed.
