Primary Health Care, Inc. is hiring a Case Manager I to assist clients living with HIV/AIDS through intake, assessment, and advocacy across PHC services and community partners.

The role requires collaboration with medical, dental, mental health, and external agencies, ensuring access to resources, transportation, and timely services. A bachelor's degree and strong communication skills are essential, with opportunities for growth within a mission-driven organization.

Qualifications

• Bachelor’s degree or equivalent combination of education/experience.
• Strong written and verbal communication skills.
• Ability to work in a team and maintain composure.
• Proficiency with Microsoft Office and database applications.
• Excellent interpersonal skills and attention to detail.
• Professional demeanor and strong organizational skills.
• Must maintain professional standards and comply with policies.
• Licenses & Certifications: Mandatory Reporter Training; Chauffeur’s license if required by state.

Responsibilities

• Completes intakes and ongoing assessment with clients to identify needs, goals and access resources.
• Monitors and documents client contacts, progress, and significant occurrences per grant guidelines and PHC policies.
• Coordinates with PHC services and external agencies for referrals and support.
• Advocates for clients with community agencies, schools and other partners.
• Ensures transportation access to appointments and assists with transportation per policy.
• Communicates with agencies on behalf of clients with proper releases of information.
• Performs outreach in community settings and serves as liaison to resources.
• Performs other duties as assigned.
