Job summary

Boston Health Care for the Homeless Program is seeking a Medical Case Manager to join a multidisciplinary HIV care team that supports patients experiencing homelessness.

You will assess barriers to care, connect patients with housing, legal and social services, and help them engage in ongoing HIV treatment. The role emphasizes compassion, collaboration, and culturally competent care in a community health setting.

Qualifications

• Strong organizational, interpersonal, written and verbal communication skills.
• Self-directed, motivated, and flexible with ability to work independently or in a team.
• Commitment to harm reduction and trauma-informed approaches.
• Equitable and culturally appropriate care for diverse populations.
• Bi-lingual fluency in Spanish is preferred.

Responsibilities

• Assess new HIV patients for financial, psycho-social, housing and other needs and develop care plans.
• Help patients engage in HIV care and treatment, coordinating with housing, legal, and social services; arrange transportation as needed.
• Collaborate with the HIV care team to develop integrated care plans and coordinate care.
• Conduct intensive outreach visits to connect patients who are newly diagnosed or out of care.
