Job summary

Community Renewal Team, Inc. is seeking a Case Manager Level 1 to coordinate intake, assessments and referrals for veterans and shelter services.

The position involves developing individualized service plans, monitoring progress and documenting outcomes in STEPS, with opportunities for counseling and group facilitation. Candidates should have an Associate’s degree or equivalent experience, active intake certification, and ability to work with diverse populations.

Qualifications

• Associate's degree or equivalent professional experience.
• Active certification in intake and eligibility for CRT programs.
• Mandatory training including confidentiality and HIPAA compliance.
• Ability to communicate effectively with diverse populations.

Responsibilities

• Complete intake and assess customer needs using the STEPS system.
• Develop individualized service plans (ISP) for each client.
• Coordinate referrals to CRT programs and community resources.
• Document progress with case notes and progress reports to agencies.
• Provide individual counseling and facilitate groups when needed.
• Conduct home visits and schedule flexible shifts including evenings/weekends.
• Maintain caseloads and ensure timely data entry and reporting.
