Job summary

PeopleShare by PROMAN is seeking a compassionate Case Manager to support veterans and their families through housing crises, using a Housing First approach and trauma-informed care to achieve lasting stability. The role involves managing a caseload, coordinating housing resources and VA benefits, and partnering with internal teams and community services to secure dependable housing.

Schedule is Monday–Friday, 8:30am–4:30pm, and pay rate is $24/hour.

Qualifications

  • Bachelor's degree or equivalent in Social Services, Human Services, Nonprofit Management, Case Management, or related field.
  • Minimum of two years of experience in social services, nonprofit programs, case management, or related human services work.
  • Experience with diverse populations and low-income households; veteran services experience valued.
  • Proficiency with Microsoft Office applications and case management databases.
  • Valid driver's license, reliable transportation, current vehicle registration, and automobile insurance.

Responsibilities

  • Manage a caseload of veteran households and provide ongoing case management services.
  • Assess eligibility, identify service needs, and develop individualized housing stability plans.
  • Connect veterans and families to housing resources, VA benefits, public assistance programs, and community services.
  • Coordinate services with internal teams and community partners to achieve successful outcomes.
  • Maintain accurate, compliant case documentation and ensure adherence to program regulations and agency policies.
  • Conduct outreach activities and cultivate referral partnerships within the community.
  • Assist eligible households with financial assistance programs and resource navigation.
  • Participate in program, community, and stakeholder meetings as needed.
