Interfaith Community Services in Escondido, CA is seeking a Case Manager II for the Recuperative Care Program. You will oversee intensive case management for at-risk Veterans and clients discharged from hospitals to support psychosocial recovery and self-sufficiency.

You will work with a multidisciplinary team, develop individualized treatment plans, coordinate housing referrals, and provide crisis intervention as needed.

Qualifikationen

• Master’s Degree in Social Work (MSW) or Master’s Degree with an emphasis in Marriage and Family Therapy (MFT) is required.
• Must be currently registered and in good standing with the California Board of Behavioral Sciences as an ASW or AMFT, or license-eligible within 6 months of hire.

Aufgaben

• Provide intensive case management for Veterans and Recuperative Care clients.
• Conduct discharge planning to housing and assist with referrals.
• Complete biopsychosocial assessments and maintain clinical documentation.
• Create and implement individual treatment plans for mental health and medical recovery.
• Provide crisis intervention as part of the treatment team.
• Participate in case consultations and team discussions.
• Maintain required clinical and administrative documentation.
• Collaborate with caregivers and community providers.
• Attend staff meetings and trainings.
• Perform additional duties as assigned.
