HealthRIGHT 360 seeks a Residential Case Manager in San Francisco to coordinate care for clients in its 815 Residential and Detox Program. You will link clients to medical, mental health, and housing services, manage aftercare plans, and guide transitions from residential treatment to lower levels of care, ensuring culturally responsive practice.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in a related field preferred.
• A minimum of 1 year of experience in care coordination, case management, or substance use disorder services.
• AOD Registration from an Accredited authority (CCAPP, CADTP, CAADE) is required.
• AOD Certification from an Accredited authority (CCAPP, CADTP, CAADE) is preferred.
• First Aid and CPR certification.
• A valid California driver’s license.

Responsabilités

• Care coordination and client advocacy within residential SUD programs.
• Referral and linkage to internal and external community resources.
• Transition planning and discharge planning to support continued recovery.
• Healthcare coordination including scheduling appointments and medication support.
• Collaboration with multidisciplinary teams to ensure quality care and compliance.
