Job summary

Ruby's Place, doing business as Restorative Pathways (RP), is seeking a full-time Case Manager for the Riley Recovery Centers program in Oakland. This hybrid role connects sober living with PHP/IOP services, coordinating care, transportation, referrals, and documentation to ensure smooth client progress.

You will work across residential, clinical, and medical teams, maintain EMR records within 24 hours, and support discharge planning while honoring confidentiality and survivor-centered

Qualifications

• High school diploma or GED required.
• Pass background screening including DOJ/FBI fingerprinting.
• Strong organizational skills and ability to manage a caseload.
• Clear, professional written and verbal communication, including EMR documentation.
• Calm, compassionate approach to clients.
• Ability to collaborate across residential, clinical, and medical teams.
• Valid CA driver’s license and reliable transportation.
• Ability to complete background checks including Live Scan and CCL documents.

Responsibilities

• Serve as resident's primary case management contact throughout their stay.
• Coordinate transportation, appointments, and documentation with residential staff.
• Own collateral contacts, referrals, and appointment coordination for PHP/IOP.
• Document case management activity in the EMR within 24 hours.
• Participate in weekly Team & Case Review meetings and report caseload status.
• Maintain client confidentiality per 42 CFR Part 2 and HIPAA.
