Job summary

HomeFirst is seeking a Case Manager for Permanent Supportive Housing in the Bay Area. This full-time, day-shift role focuses on delivering client-centered case management and connecting participants to housing resources within Santa Clara County.

The position requires 2 years in social services, and a BA in Social Work is preferred. Bilingual Spanish is a plus; a CA driver license is recommended. You will document in HMIS and collaborate with partners to support client goals.

Qualifications

• 2 years of paid or volunteer experience in social services.
• BA in Social Work preferred, not required.

Responsibilities

• Provide case management and direct services to clients.
• Coordinate with team members remotely and on-site.
• Collaborate with community partners to link clients to services.
• Maintain HMIS records with timely progress notes.
• Report suspected abuse and maintain documentation.
• Assist with life skills workshops and transportation as needed.
