Job summary
Telecare Corporation seeks a dedicated Case Manager/Housing Specialist in San Diego to support individuals with mental illness in securing and maintaining independent living in the community. Under direct supervision you will coordinate housing services, transportation, and linkages to community resources to promote housing stability.
Full-time position in a comprehensive behavioral health program, offering PTO, holidays, CEUs, tuition discounts, and a full benefits package.
Qualifications
- High School diploma or GED or higher with experience in mental health or community services.
- At least 1 year of direct service experience as Housing Specialist.
- Valid driver's license and personal vehicle insurance in applicant's name.
- Willingness to use personal vehicle to transport clients to appointments.
Responsibilities
- Provide primary support and case management for individuals with mental illness and co-occurring disorders.
- Assist clients to live as independently as possible in their chosen community.
- Coordinate transportation and linkages to community resources to promote housing stability.
