PATH in California seeks a Case Manager – Care Coordinator to support clients experiencing homelessness. You will conduct intake, assess needs, and develop individualized service plans that address housing goals, income, and personal health needs.

Qualifications

• All levels of education and experience are welcome.
• Two years of experience in healthcare, case management, homeless services, or related fields.
• Mandatory: CPR/First Aid training and ability to obtain it; Tuberculosis Test, Background Screening, Drug Test as conditions of hire.

Responsibilities

• Conduct intake and individualized needs assessment and develop an Integrated Service Plan addressing housing goals, income, and personal needs.
• Provide referrals and supports to resources to help clients achieve Service Plan goals.
• Complete program specific assessments (e.g., VI‑SPDAT).
• Evaluate progress and conduct 90‑day reviews with clients after intake.
• Maintain complete, accurate client files and documentation per PATH policies.
