Job summary

PATH is seeking an Outreach Case Manager for the Mobile Homeless Response Team in San Diego. This full-time, non-exempt role supports clients through housing transitions, providing intake and Individualized Service Plan development and coordinating access to healthcare, social services, and housing resources.

You will work on-site at PATH, contribute to PATH’s mission to prevent homelessness, and help clients navigate a continuum of services across the state of California.

Qualifications

• Intake and needs assessment to develop ISP addressing housing goals and resources
• Referrals and coordination to help participants access ISP goals
• Completion of program assessments (e.g. VI-SPDAT)
• Ability to evaluate progress and conduct 90-day reviews with participants
