Job summary

The Colorado Coalition for the Homeless seeks a Case Manager I to work with diverse clients facing homelessness. You will develop individualized care plans, coordinate services with partners, monitor progress, and advocate for resources, including housing and health supports.

Responsibilities include crisis intervention, referrals, and thorough documentation in the EHR, with a focus on trauma-informed, housing-first practices and collaborative teamwork across the organization.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Colorado driver’s license within 30 days of hire.
• 1–2 years experience with homelessness or mental health challenges preferred.

Responsibilities

• Develop and implement care plans for clients.
• Coordinate services and resources across partners.
• Monitor client progress and outcomes.
• Provide advocacy and support to residents.
• Facilitate referrals to external services and housing.
• Maintain case documentation and data in the EHR.
