Qualifications

• Experience developing individualized, long-term recovery plans and accessing resources.

Responsibilities

• Conduct interviews and assess client needs to develop comprehensive, long-term plans.
• Advocate for clients to obtain needed services and resources.
• Maintain accurate records and prepare case presentations as required.
• Coordinate with external agencies and housing/financial assistance programs.
• Monitor client progress and adjust plans as needed.
