Job summary

The Salvation Army A Georgia Corp seeks a dedicated Case Manager to support clients in a short-term life management program. You will interview applicants, develop intake and case plans, and guide clients toward housing and service referrals, while documenting progress in HMIS.

You will maintain accurate records, coordinate life-management classes, and uphold standards of conduct and confidentiality in a supportive office environment.

Qualifications

• Two years of college coursework in social work, behavioral science, or a related field.
• Two years’ experience in a social/public service environment.

Responsibilities

• Interview applicants to determine eligibility for program enrollment.
• Develop intake and case plans and complete necessary paperwork.
• Maintain case records and HMIS data.
• Meet regularly with clients to discuss progress.
• Assist clients with housing and service referrals.
• Prepare discharge plans and follow-up activities.
