Job summary

Salvation Army is seeking a full-time Case Manager to deliver structured, long-term services to clients in programs such as Pathway of Hope, Veterans, and Rapid Re-Housing. You will interview, assess needs, set goals, and monitor progress with counseling and advocacy, while ensuring HMIS data accuracy and HUD grant compliance.

The role requires a Bachelor's degree in social work or related field and at least three years of direct case management experience, plus a valid state driver's license.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Minimum of three years' direct case management experience.
• Valid State Driver's License required.
• Experience with HMIS or similar case-management systems preferred.

Responsibilities

• Interviews applicants to determine eligibility for Pathway of Hope program enrollment.
• Develops long-term program goals and evaluates client progress through counseling.
• Maintains case records and HMIS entries for assigned clients.
• Ensures HUD grant compliance and coordinates services for Veterans and VA residents.
• Advocates for clients to access needed services and resources.
