Job summary

The role requires a Bachelor's degree in a related field and at least three years of direct case management experience, plus a valid state driver’s license.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or a related field.
• Three years progressively responsible experience providing direct case management social services.
• Valid State Driver's License.

Responsibilities

• Interviews applicants to determine eligibility for Pathway of Hope program enrollment.
• Establishes long-term program goals for eligible clients and counsels on behavioral modification.
• Maintains case records and HMIS entries for assigned clients; ensures data accuracy.
