Job summary

The position requires coordination with community partners, documentation in HMIS, and ensuring compliance with funding requirements. A bachelor's degree and driver’s license are expected for applicants.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field.
• Three years of progressive experience in direct case management.
• Experience in accessing needs and developing long-term recovery plans.
• Valid State Driver's License.

Responsibilities

• Interviews clients to determine eligibility for services and documents income, family and work history.
• Develops comprehensive program plans and goals for clients in a life management program.
• Provides mentoring and counseling to clients and staff; advocates for client access to services.
• Documents case progress and outcomes; maintains HMIS records and monthly reports.
• Coordinates housing, food pantry, and transportation assistance as needed.
• Attends community meetings and coordinates with partner agencies to support clients.
