Job summary

IMPACCT Brooklyn is seeking a full-time Social Services Case Manager to support a caseload of chronically homeless adults with complex needs in Brooklyn, NY. You will deliver intensive case management, coordinate with health and social services, and help clients access benefits and housing resources.

The role requires a 4-year degree and preferably 5+ years in case management, with strong written and verbal communication skills and a compassionate, solution-focused approach.

Qualifications

• Bachelor’s degree in social work, psychology, or mental health (MSW preferred).
• 5+ years of case management experience.
• Residential setting experience preferred.
• Strong written and verbal communication skills.
• Working knowledge of MS Office (email, Word, PowerPoint).
• Bi-lingual (Spanish) preferred.

Responsibilities

• Manage a caseload of 12 chronically homeless individuals with special needs.
• Provide person-centered best practice due diligence intensive case management for clients with co-occurring disorders.
• Develop quarterly treatment service plans and objectives; complete bi-annual assessments and annual psychosocial.
• Advocate for benefits, entitlements, and legal services for clients.
• Provide crisis intervention and referrals for psychiatric assessment as needed.
• Coordinate care with medical, mental health, substance abuse, and supportive services providers.
• Assist clients in achieving independent living and document all activities.
• Maintain confidentiality and integrity of client information.
