Job summary

IMPACCT Brooklyn is seeking a full-time Social Services Case Manager to support a caseload of 12 chronically homeless individuals with complex needs. You will provide person-centered, intensive case management, coordinate medical, mental health, and housing services, and develop quarterly treatment/service plans.

Strong written and verbal communication skills are required; bilingual Spanish is preferred. You will document all contacts, advocate for benefits and legal services, participate in

Qualifications

• Bachelor’s degree in social work, psychology, or mental health (MSW preferred).
• 5+ years of case management experience may be considered.
• Residential setting experience preferred.
• Bi-lingual (Spanish) preferred.

Responsibilities

• Manage a caseload of 12 chronically homeless individuals with special needs.
• Provide person-centered, best-practice case management services.
• Assist clients in advocating for benefits, entitlements and legal services; provide crisis intervention as needed.
• Coordinate care with medical, mental health, substance use, and supportive services providers.
• Present client needs and strengths in interdisciplinary care planning meetings.
• Maintain confidentiality and document all contacts and services in compliance with contract and agency standards.
