TheCaseManager providesdirect case management services whileengaging homelessindividualswith appropriate concern for their social, mental health, substance abuse and medical problems in order to promote client stability and empower them to reach their fullest potential.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Completesintakes for new clients and providesanorientation to the program and shelter rules and regulations
- Reviewslogbooks atthestart of each shift toensure that all clients follow-upsareconducted as required
- Participatesas part of a team and regularlycollaborateswith multidisciplinary staff to monitor clients’progress
- Developsa comprehensive psychosocial assessment to fully identify and address theclient's needs
- Collaborateswith internal staff to monitor clients’ medication compliance and address episodes of non-adherence
- Establishesan individualized intervention/treatment plan or strategy to promote identified goals
- Providesadvocacy for individual clients around entitlements and services
- Providescase management, including individual counseling, crisis intervention, conflict resolution, advocacy,psychoeducation, coordination of services and appropriate follow-up on all referrals
- Usesmotivational interviewing and seekssafety modalities to counsel clientsto improve overalleffectiveness
- Encouragesand providesongoing support so that each client might assume an active role in his or her owntreatment plan tothe maximum extent possible
- Facilitatesclinical, educational, supportive and/or recreation groups to address problemareas that clientsexperience in working towards independent living
- Maintainsrequired documentation in case records,in written and computerdatabases(SCIMS, AWARDS) andupdatesclientrecords in accordance with governmental and agency policy
- CompletesHousing applications (both written and computer generated) for clients based on their individual needsandfunctioning
- Escort clients as needed.
- Facilitatesand assistswith clients' transfers into independent living
- Participatesin weekly case conferences with both internal departments and externalproviders
- Participatesin regularsupervision, team meetings,caseconferences and staff trainings
- Maintainsa flexible scheduleincludingevening and weekend shifts
- Performsother duties asit relates to case management
JOBQUALIFICATIONS
Educational Requirements
- Bachelordegree in a Social Work, Psychology,Counselingoranotherrelated fieldpreferred
Experience
- Three (3) to five (5) yearsofcase management experience with the homeless, mentally ill, substance abuse and/orlow-incomepopulations preferred
Skills Set
- Strong relationship-building skills required
- Ability to work independently but know when to raise issues tothesupervisor
- Excellent organizational and time management skills necessary
- Ability to effectively multi-taskandfollow upon a timely basis
- Strong PC skills required including Word and Excel
Languages
- Bilingual in Englishand Spanish strongly preferred
