Job summary

IMPACCT Brooklyn seeks a dedicated Case Manager to oversee a caseload of vulnerable adults in a supportive housing setting. You will provide intensive, person-centered services to clients with co-occurring mental health and substance use disorders, coordinating with medical and community providers to secure benefits and housing stability.

You will document all interactions, participate in interdisciplinary planning, and maintain HMIS records.

Qualifications

  • Bachelor’s degree in social work, human services, psychology, or related field.
  • 2+ years of case management experience serving vulnerable populations.
  • Knowledge of Medicaid, SSI/SSDI, HASA benefits and entitlements.
  • Experience with HIV/AIDS, mental illness, and substance use a plus.
  • HMIS database experience is a plus.

Responsibilities

  • Manage a caseload of 17 chronically homeless individuals living with HIV/AIDS.
  • Provide person-centered, intensive case management for co-occurring disorders.
  • Develop bi-annual treatment service plans and conduct assessments.
  • Assist clients in obtaining benefits, advocate with providers, and coordinate care.
  • Document services and submit progress notes to the Director weekly.

The ideal candidate will prioritize client health and safety, demonstrate strong teamwork skills, and be responsible, reliable, and accountable. If you are committed to making a difference in the lives of those in need, we encourage you to apply.**Essential Duties and Responsibilities*** Manage a caseload of 17 chronically homeless individuals living with HIV/AIDS, who may have a history of substance use and/or mental illness* Adhere to the Social Work Code of Ethics* Provide person-centered, comprehensive intensive case management services to clients with co-occurring mental health and substance use disorders* Develop clients’ self-identified bi-annual treatment service plans and objectives; complete bi-annual assessments of service needs and an annual psychosocial assessment* Assist clients in advocating and obtaining benefits, entitlements and legal services;* Provide crisis intervention services and if necessary, refer clients for psychiatric assessment* Provide collaborative care coordination with service providers for clients, including primary and ongoing medical, mental health, substance abuse and supportive services* Assist clients in obtaining and, if possible, achieving independent living;* Provide weekly face-to-face case management services and monthly home inspections.* Maintain community partnerships with medical providers, treatment centers, and detox and rehabilitation programs* Present psychosocial needs, strengths, and limitations of all assigned clients during interdisciplinary care planning meetings and case conferences* Ensure the integrity and confidentiality of all client information;* Participate in 6 job development training courses per year* Documentation of all contact and services in compliance with contract and agency standards and provide all weekly contact progress notes to Director by COB Friday* Provide accurate and up-to-date client documentation in the HMIS Database System* Facilitate group sessions* Perform other duties as required**Knowledge*** Knowledge of ethical standards, principles, and practices of social work, counseling, and human behavior.* Knowledge of Person-Centered and strengths-based methodology.* Knowledge of motivational interviewing and other informational gathering techniques.* Knowledge of the theories of group interaction and therapeutic intervention.* Knowledge of geriatric population* Knowledge of mental illness and common psychiatric diagnosis* Knowledge of substance abuse issues/concerns**Abilities*** Ability to engage, motivate, and build trust with clients living with HIV/AIDS, serious mental illness, and substance use disorders.* Ability to write meaningful, concise, and accurate progress notes and correspondence.* Strong communication and active listening skills, with the ability to advocate effectively on clients’ behalf with providers, government agencies, and community partners.* Compassionate, client-centered approach to supporting individuals in crisis or navigating difficult transitions.* Strong organizational and time-management skills to manage a full caseload, meet documentation deadlines, and coordinate with multiple providers.* Critical thinking and problem-solving skills to help clients identify solutions and achieve their goals.## Qualifications**Minimum Qualifications*** Bachelor’s degree in social work, human services, psychology, or a related field preferred; associate’s degree with 3 years’ case management experience will be considered* Minimum 2 years of case management experience serving vulnerable populations; supportive housing or residential setting preferred* Knowledge of case management principles, community resources, and benefits and entitlement systems (e.g., Medicaid, SSI/SSDI, HASA)* Experience working with individuals living with HIV/AIDS, mental illness, and substance use issues a plus* Working knowledge of computer programs including email, Microsoft Word, and PowerPoint; HMIS database experience a plus* Must be willing to work a flexible schedule.* Fluency in the English language is required; Spanish-speaking ability is preferred.**Physical Demands and Work Environment** This position is in a social services supportive housing environment, based on-site at a supportive housing building. The role involves a mix of administrative tasks and direct engagement with program participants, including formerly homeless individuals with mental illness, substance use disorders, and health conditions. The role requires the ability to sit or stand for long periods, occasionally lift up to 25 pounds, move around the building, and travel between IMPACCT Brooklyn sites. Reasonable accommodations are available for individuals with disabilities. The work environment is an office within a residential supportive housing setting, with noise levels varying from moderate to high based on time and client activity. The employee will regularly interact face-to-face with clients, some potentially in crisis or exhibiting mental health or substance use behaviors, and must follow safety, de-escalation, and confidentiality protocols.
