Job summary

Qualifications

• Bachelor's Degree required.
• One year related experience and/or training.
• Working knowledge of Microsoft Office Suite and other technology.
• Excellent communication and listening skills.

• Able to multi-task and work independently with a great attention to detail.

Responsibilities

• Conduct intakes and screen clients with presenting problems.
• Provide information on benefits, entitlements, programs, and case assistance; assist in accessing services and coordinating referrals.
• Apply and recertify clients for entitlements and benefits.
• Arrange for provision of services and coordinate care plans.
• Maintain respectful, clear communication with clients and families.
• Maintain complete case notes and data in the program system; monitor reassessment and contacts.
• Prepare and maintain required documentation; compile statistics and reports.
• Participate in supervision and staff meetings; share information timely.
• Assess social, emotional, and environmental status of clients in various settings.
• Develop, implement, and follow up on care plans; provide ongoing client contact and supportive counseling.
• May supervise social work students and arrange translation services as needed.
