Job summary

Job Board in New York, NY invites a Case Manager for its Community Guardianship Program. This full-time, hybrid role assesses clients' needs, screens for services, and coordinates entitlements and benefits while maintaining case notes and care plans.

You will conduct intakes, help access services, document outcomes, supervise students as needed, and collaborate with agencies. A Bachelor's in Social Work and at least one year of related experience are required; proficient with Microsoft Office

Qualifications

Bachelor's Degree in Social Work or related field.
One year related experience and/or training.
Working knowledge of Microsoft Office Suite and other technology.
Excellent communication and listening skills.
Excellent customer service skills with a focus on treating clients with respect and dignity.
Able to multi-task and work independently with a great attention to detail.

Responsibilities

Assess clients' needs and eligibility for in-home services and benefits.
Conduct intakes and screen clients; provide information on entitlements and programs.
Coordinate services, document case notes, and maintain case files.
Prepare reports and disseminate information; supervise students as needed.
Arrange translation services and advocate with other agencies as required.
