Job summary

Selfhelp is seeking a Social Worker to assess clients’ needs for in-home services and entitlements, manage ongoing casework, and help clients access programs. The role includes intake screening, care plan development, documentation, and coordination with service providers.

The position requires strong communication and customer-service skills, attention to detail, and the ability to work independently, with potential for remote work 2–3 days per week in a hybrid setting.

Qualifications

• Bachelor's Degree required.
• One year related experience and/or training.
• Excellent communication and listening skills.
• Excellent customer service skills with respect and dignity for clients.
• Able to multi-task and work independently with attention to detail.

Responsibilities

• Assess clients' needs and eligibility for in-home services, entitlements and benefits, and manage ongoing case work and care plan management.
• Conduct intakes and screen clients with presenting problems, if required.
• Provide information on benefits, entitlements, programs, and case assistance; assist in accessing services and advocate with other agencies.
• Apply and recertify clients for entitlements and benefits.
• Arrange for the provision of services and translate services when needed.
• Maintain complete and accurate case notes in the data system and prepare required documentation.
• Prepare reports and disseminate information as necessary.
• Participate in supervision and staff meetings; exchange information promptly.
• Assess social, emotional, and environmental status; develop and follow up on care plans.
• Provide continuing contact and supportive counseling to clients.
