Job summary

BronxWorks is seeking a dedicated outreach and case management professional to support residents across multiple programs in the Bronx. You will guide clients through intake, assessment, service planning, referrals, advocacy, counseling, and follow-up while managing a caseload and documenting progress.

The role emphasizes collaboration with social services, mental health, medical, employment, and housing partners; requires strong communication and organizational skills, a willingness to travel

Qualifications

• Bachelor’s degree with case management experience or High School Diploma with 3 years of case management experience.
• Experience using Access HRA and securing HRA entitlements and benefits preferred.
• Mental health background preferred.
• Crisis prevention and mental health first aid training is required within first 6 month.
• Strong oral and written communication, time management and organizational skills are necessary.

Responsibilities

• Assist with the outreach and engagement of residents of 2605 Sedgwick and Bronx Park Phases.
• Assist with NYSERDA Green Energy program- program participant and follow-up services.
• Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up
• Manage a caseload
• Monitor and document client progress toward service plan goals
• Document all interactions with, or on behalf of, clients
• Maintain familiarity with program resources available on- and off-site
• Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers
• Complete program reports on a monthly basis, or more frequently as required
• Act as program contact person for inquiries concerning clients
• Provide collaboration and support with securing benefits, especially as it relates to access to benefits services and eviction prevention efforts
• Report to, and meet with, supervisor on a monthly basis
• Conduct home visits and other field work, as required
• Coordinate and facilitate monthly activities that fit the needs of the site
• Perform additional administrative duties as assigned
• Assist with community events as needed and serve as a liaison with community vendors
