Job summary

Adapt Integrated Health Care in Gold Beach, Oregon, is seeking a qualified Case Manager to join the Crisis team. This full-time role runs Monday–Friday, 8:00 am–5:00 pm, and includes crisis intervention support under supervision.

You will provide case management, individual and group skills training, and coordinate with a multidisciplinary treatment team to promote recovery and community living. A bachelor’s degree, two years of social service or mental health experience, and a valid driver’s

Qualifications

• Bachelor's degree in a social service and/or mental health field or equivalent.
• QMHA registration eligibility through MHACBO.

Responsibilities

• Provides case management, individual and group skills training, and crisis intervention as directed by a QMHP.
• Coordinates with treatment teams and partners to ensure service continuity.
• Maintains records, reports, and documentation in compliance with policy.
• Transports clients and supports to services as needed.
• Supports crisis response and collaboration with law enforcement and community providers.
• Responds to crisis calls with resources and safety planning under supervision.
• Engages families and clients in treatment plans and supports.
• Participates in emergency response duties as required.
