Job summary

Evergreen Treatment Services in Seattle seeks a Case Manager for the HCHN program, Monday–Friday 8–5. The role focuses on direct client care, intensive case management, and system navigation for unhoused individuals with behavioral health needs.

Responsibilities include comprehensive assessments, service planning, and collaboration with medical, housing, and social services to improve stability and access to treatment. Bilingual skills are a plus.

Qualifications

• Credential: SUDP/SUDPT and MSW highly desirable.
• Education: HS diploma or equivalent; BA/BS preferred.
• Experience: 3+ years in social work/outreach; harm reduction experience essential.

Responsibilities

• Direct Client Care, Street-Based Outreach & Intensive Case Management (70%).
• Care Coordination across medical, behavioral health, housing, and social services (20%).
• Documentation, compliance, and program participation (10%).
• Maintain client records and develop individualized service plans.
