Job summary

Adapt Integrated Health Care in Gold Beach, Oregon, seeks a qualified Case Manager to join the Crisis team. This full-time role runs Monday–Friday, 8:00 am–5:00 pm, providing crisis intervention, case management, and skills training under QMHP supervision.

You will coordinate with multidisciplinary teams, document ISSP plans, transport clients as needed, and support re-entry into the community. A bachelor’s degree with two years’ relevant experience and a valid driver’s license are required.

Qualifications

• Bachelor’s degree and two years’ experience in a social service and/or mental health setting.
• Valid Driver’s License.
• Eligible to register as a QMHA through MHACBO.

Responsibilities

• Provides case management, individual and group skills training, mentoring and crisis intervention as directed by a QMHP and ISSP.
• Coordinate interventions with treatment team and foster providers to ensure outcomes and problem solving.
• Maintain and complete required records, reports and documentation as policy and law.
• Transport individuals and supports to/from services.
• Support crisis programs and coordinate with law enforcement and hospitals for re-entry.
• Respond to crisis calls with the mobile crisis team and provide safety planning.
• Collaborate with individuals, families and community partners receiving services.
• Emergency Response: report for duty during health emergency activation.
